Deaf or hard of hearing
Use the contact form below and tell us to reply by email or text rather than calling. Texas Relay is available by dialing 711, and you are welcome to bring an interpreter to the suite.
Blind or low vision
Ask us for arrival instructions and directions in large print or plain text. Service animals are welcome, and staff can guide you from the entrance.
In an emergency, call 911. This website and these forms are not monitored around the clock and are not a secure way to send medical information. Never send protected health information through them.
